My take is this was a fifth wheel pin box went through the bed floor. Having said that, the damage would have been very minimal if this was a steel body truck.

A fifth wheel would have crushed the bed sides before going thru the bed floor...
Well, looks like someone forgot to double check the jaws were locked and took off, and dropped the trailer. OOPS. I know my RV would hit the bed before the bed rails and would probably do the exact same thing if I forgot that one important step.
No way a 5th wheel pinbox could do that without contacting the bedsides unless you have over 12" of clearance between your 5th wheel and bedrails. Look how high the 5th wheel plate is above the bed floor and the pinbox would have to travel that whole distance to the bed floor unencumbered. Which means you would need at least the height of your 5th wheel hitch for bedrail clearance with your 5th wheel hitched up.
Missing tailgate is also a tall tell sign. The landing gear is what probably saved the bed rails or it would have been a complete military grade disaster.
Pin box slipped out the hitch on my parents 05 Chevy years ago and the bed sides never got damaged Then again I'm talking about a steel bed that doesn't puncture and tear. Having said that just because a fifth wheel hits the bed rails doesn't mean it will cause visual damage.
on those plastic caps it will. They are cheap. I've seen an 03-07 chevy have the 5th wheel slip off the 5th wheel plate and it crushed the steel bed sides. That was a 2009 5th wheel around 12k lbs. You cannot logically come to the conclusion that a 5th wheel pinbox did that hole and bent that flap of aluminum that far down and the bed sides are spotless.

Bottom line a truck bed should not fail like this. A complete epic military grade failure.

you might be wrong bubba. There is more damage than you might first notice. I think fish on one is right and even Arod (guy filming) indicated it came back thru the bed
the bed rail did get hit pretty hard. Enough to case the separation of the top plastic rails and gashed it as well. See attached pics.
