rusting Floorboards

I have a '89 truck and the floorboards are in pretty bad shape. I have heard that you can just grind most of the rust off, but what do I do where it has gone all the way through. Should I cut it out and weld a patch in place or what? Any and all ideas would be appreciated.

that would work. get all the rust out and cut out the worse parts
weld new metal in or pop rivet. if you rivet them in use seam sealer all the way around.
