New problem. I'm getting severe vibrations in my shifting stick, clutch pedal and 4wd stick especially when over 50mph. At 60 mph the shaking is pretty gnarly. It happens whether I'm giving it fuel or not. It also happens in neutral at those speeds as well. My transfer case has been leaking like a sieve in several places for several weeks now and I'm putting an order in for an overhaul kit on that. I'm kinda leaning towards something off-balance with the flywheel, but not sure at this point. Any ideas where to start?
I'd check my mounts first. Motor and transmission. And i'd stop driving that thing over 40mph until you find the problem in case it decides to let go on you.
You can check the motor/transmission mounts with a buddy. Lift the hood and watch the engine when your friend shifts into drive and reverse. If the mounts are bad the engine will rock significantly when shifting into gear.
Does the truck make the noise only when moving? Is it noisy when you rev the engine in park?
Sounding like an "under the truck" issue. I'd get on a creeper and go from back to front and do a visual as I checked for anything with slack in it.
Being it vibrates when in neutral when going 40 mph, I'd focus first on the drive line maybe even the tires.
So far everything looks good (without breaking into the transmission and transfer case), even the flywheel seems locked in place. We did notice a large tear in the exhaust pipe located directly next to the transmission. The tear goes pretty much around the entire pipe. I'm wondering if the torn exhaust pipe is causing the vibrations at the higher speeds? Or the tear is the result of whatever is causing the vibrations?
Certainly appears to be stress cracks rather than corrosion.
Do you have the capability to lift the truck off the ground and safely block it up ?
That would allow you to run the drive train up to speed and make some observations as to where the vibration is coming from.
Things to look for... Bent wheel / rim, out of round tire, driveshaft bent, etc.
If nothing is obvious then disconnect the drive shaft and run it up to speed. That will narrow down your search area.

Thanks for all the recommendations. Still working on this. Everything checks out fine so far. I was at my wits end and it went to 3 different mechanics, all of who cannot figure it out either. Tires are all perfectly balanced, no broken belts. At this point I removed the driveshaft and I'm having the balance checked and getting new U-joints just to make sure.
I'm also in the middle of a complete transfer case rebuild as fluid is leaking from every possible place there. I have two questions that perhaps someone can shed some light on regarding that...
First, does anyone know what the purpose of the hose that's located on the exterior of the front of the transfer case is? Attached pic, the previous owner has it capped-off for some reason.
Secondly, looking for what size socket I would need to remove the large nut that's also located on the front of the case. It looks to be 1-1/4", but just wanted to verify before heading off to buy one, or two, or three of the wrong sizes. In the pic the nut/bolt is drowning in a pool of PB blaster because I'm expecting lots of headaches getting it off.
