332 is insufficient flow. It can either be a stored code or show up in the KOER test. Which is it and how did you delete the EGR system? It still needs to be electronically connected to the solenoid or else it will set a code.
452 vehicle speed sensor signal fault. According to my book that will only show up as a stored code. Did you erase the codes after replacing the sensor?
628 excessive converter slippage. Is this a stored code or did you get it from the engine off test?
One possibility for delayed engagement is low fluid. Have you checked the transmission fluid level? It has to be checked at operating temp.
It’s hard to say what the cold start problem is without doing testing. It could be the temperature sensor providing a false engine temp.
Code 332: The same code # will mean different things for different years and vehicles. Insufficient flow only applies to vehicles with the DPFE egr system which is '96 only for these trucks. This '91 would have an EGR position sensor where the code definition is "valve did not open" when commanded, but with the EGR deleted there is no way to clear this code.
Code 452: This truck should not have the electronic speedo cluster... it should be mechanically driven via a cable from the trans or t-case. That is good news as there is no PSOM between the speed sensor and the PCM. But that also means the rear axle sensor does not serve as the VSS, the output on the trans or t-case should be a combo electrical/mechanical device such as this...
Code 628: Could just be leftover from the original problem, have you cleared codes to see what cones back? Stored codes do not self erase.

With a code reader or by disconnecting the battery for 5mins, but that also erases all adaptive learing the computer has built up for the engine and trans.
