Form FP 8096-B?

Anybody out their know what is and were I can find Form 8096-B, this form is mentioned on my Motorcraft 1973 / 79 Truck Master Parts And Accessories catalog (100-500 series)

What is the contect of the mention of this form? Can you post the entire page where this appears? Or the page number?

Oh ok, The Motorcraft 1973/79 Truck Master Parts and Accessories catalog are the forms themselves- part FPS 8096-A and part FPS 8096-B. Thank you for the help.

I believe dlburch was able to answer my question, I don't remember exactly what page it was. I just know that every time I stumbled with a reading that required me to look at form 8096-A -I could never find it.
