Swapped #1 & #5 with #3 & #7? What mods do you have? How's the idle, any loping/knocking/shaking? How's the valley look, any drips or puddles? For that matter does the truck "mark its territory?"
Is the IPR DC% really that steady and low? Mine usually runs around 8-11% at idle and moves a little, even 1/2%, something. Don't know just raises my brow a little.
What's it read while driving. Have you checked out the pigtail on the IPR? Open the harness back a little look for worn or soft spots on the insulation. Had mine short out awhile back about 4-6 inches back from the plug.

The IPR is how the PCM controls the injectors, and the DC% tells you what what it's asking for. Since the injectors use high pressure oil to deliver fuel, then the higher the # the more fuel delivered. So the first question always asked when the engine isn't running right is ICP, IPR, DC%, IPR volts. So yeah an interruption in that signal does cause some problems. Your DC is lower that I've seen personally, but I'm no expert, so the pigtail is worth a look at least. Doesn't cost anything to look. The CCT codes are curious, but maybe not directly related. If the wiring inspection doesn't lead anywhere maybe try moving those injectors around. Are the rest original? Don't know but maybe swapping two worn out sticks for new while the rest are borderline could cause contribution imbalance.(totally spit balling)
All the injectors are fairly new. I'm going to check the pigtail on the ipr tomorrow and see if any wires are chafed or bare somewhere. Seriously at a loss right now... don't want to keep throwing parts at this thing. Any ground wires I should pay any special attention to or anything?
Alot of codes. Let's see what's real. Now that you've captured the codes, use Forscan to clear them. Idle the truck for a few minutes, and read the codes again. Let's see what comes back.
Can you run your CCT again and see if you still have P0263 and P0275, now that you've changed out injectors 1 and 5.
Need to get some believable data on Forscan. Suggest picking just one ICP PID and set it to read PSI, not kPa. IPR% is not right, so we need to figure that one out. Should be about 17% cold idle, and come down to 8-12% when warmed up, like udsuth78 said.
Standing back and just looking at this - it reminds me of electrical noise. I wonder if the battery connections and grounds are clean and tight. Maybe check your battery cables and connections at both ends for good contact.
