Engine/injector issue

Finally got the problem figured out. My truck is a 2000 F450 cab and chassis and on these trucks the fuel tank has a coating on the inside that delaminates. I had chunks in the fuel filter. After the shop replaced the fuel tank, fuel filter, fuel pump and cleaned the lines she runs again. Problem solved

I like the simple solution. We often start at the basics like oil and fuel level, fuel pressure, batteries, cables and grounds in good shape, and then dive into the codes, and check ICP and IPR readings. Glad you got this nailed.
What do we make of all the codes for High side open Bank #1 and #2, etc? Old codes that were not cleared?
