Yes, our 2001 has done that a couple times. First time was a cracked PCV vacuum line. Then a few months later, cleaned the MAF sensor, and it started doing it again. It was the same plug you are talking about on yours, kept falling out and causing it to suck unmetered air into engine. Had the same codes as you, but also got misfire codes. After I found it, I think I reoriented the wiring a little, maybe the whole MAF sensor I don't remember, and it seemed that the plug wasn't trying to come out anymore. I still went ahead and wrapped some duct tape around it, just in case. Haven't had a single issue with it since then.
Update: -10 last night and 21 now and the truck fired right up and idled perfectly while we moved it around clearing snow off the driveway. Idled about 45 minutes.

What are those numbers for?
If it's fuel pressure, you're way too low, should be 30+ psi.
At 21 psi it might idle, but wouldn't drive well under load.

Hi Folks,
Our project was weather delayed until last Sunday. Prior to the project we had to rework some of the two way radios and computers that are installed in it and we ended up letting it idle for almost 7 hours on Saturday and it idled without any issues.
On Sunday we drove it 476 miles and it idled for a couple of hours between driving around Eastern Colorado and Southern Nebraska dirt roads.
