Noticed this unused sensor on my 2002 E350 6.8 today, when I was replacing the IACV:

That appears to be an EGR control solenoid/motor.
Disconnected due to improper maintenance, neglect, and/or modification.

Sam I Am you seem very confident of your answer. I'm not as confident about mine but I'll throw it in anyway: isn't that for the evap system?

also why aren't we talking about the bigger opening just next to it.

The opening is the IACV. I pulled the hose to replace it.

I vote evap system.

This could be a source for a vacuum leak if not plugged correctly.

Here is my "guess"
2002 FORD E-350 6.8L V10 EGR Valve Control Solenoid / Motor | RockAuto

and here are 2 possibilities for your "guess":
2002 FORD E-350 6.8L V10 Vapor Canister Purge Valve / Solenoid | RockAuto

2002 FORD E-350 6.8L V10 Vapor Canister Vent Valve / Solenoid | RockAuto

Yup, EGR control - is there any EGR plumbing on the left-side manifold?
