It is really weird how most crew-cab fords from the early to late 90's had rear headrests and my 1999 doesn't have it any more, while my old 95 K2500 x-cab had some really nice adjustable ones. Did Ford conclude after all their experience through most of the 90s that rear headrests were not necessary? Most Ford Cars (focus,taurus,crown vics, grand marquis, town cars, etc) don't have them, but Ford european cars do (most jaguars, volvos and land rovers, but the aston martins don't).
I'm thinking about adapting some if not necessary for safety (the rear glass is pretty strong) at least for comfort.
Any suggestions?

NOTHING can sustain an 85 MPH rear end crash ! Well MAYBE an Abrams tank,,,,
but the fuel bills and damage to the roadways MIGHT be a deterent for them
And most of the blame MIGHT lay with the drunk going 85 mph and not with Ford !
Well... if we all drove Abrams tanks ... why would we need/care about roads?
We had three (3) headrests added to our new '03 X right after we had it delivered. I don't know how it was done, but it cost $625. The color match is excellent, but the upholstery work is not "perfect". An acceptable compromise.
We had the 3 headrests added at Kenny's Auto Accessories here in Tulsa (Kenny's Auto Accessories) back in 2003 as noted above. I just had them do it again for our 2005, and did not want to recommend them until I saw the quality of their work. My wife approves and it seems to give the "Midas" (our 05 Ex) a more finished and comfortable appearance.
They told me they used the headrests from a Mercury Villager minivan. It took them over 2 weeks to find used headrests so they would have a place to start. They reshaped the headrests to match 2nd row and obtained vinyl to match. The only thing a bit off is the color of the plastic adjuster clips on the top of the seat. I may look for vinyl paint to make these match. Cost this time: $400. If you want to discuss with them, you might want to ask for Kenneth.

My install photos are in my gallery. if I was to do it again, I would go ahead and drill the holes next right on top of the supports. (See pics to understand). $400 is a LOT of dough but hard to put a price on a kid's head.
My headrests were from one of my Mazda 5s. They are tall enough that I didn't see the need to make them adjustable.
