Weird I just bought one for a 5.4. Searching NAPA, Autozone etc was worthless. Had to go to an online OEM parts store and get the number to search Google. Frustrating!
Had the same problem on my 01 F350/V-10. ould not find and I had heard it comes with the valve cover but the cover is no longer available! Cleaned mine up really god with Brake-Kleen and J-B welded it all the way around. Lasted for 5 years till I bought a different truck. It will set a code if bad enough to show lean on both banks.
Delete the feedback going into the air intake, vent that crap out the underside of truck. Any similar oil and temp rated hose from parts store will work. Of course, if the wife complains about the smell, just vent it back into the intake.

No matter what Gates/Rock Auto, et al. say, that is not the crankcase vent hose connector found on my 2002 V10.
I was able to track down a possible replacement locally, at a salvage yard. Problem is it is from a 2000 V10, and the replacement consists of an entire valve cover. In that year the crankcase vent connector was a steel nipple that was press fitted into the valve cover. By 2002 Ford modified the set up by making the connector a plastic nipple that snapped in. Cheaper production cost I presume?
I can get that valve cover for $50 if I remove it, and for $100 if I have them remove it. Too much money and too much work as far as I am concerned.
So, it now appears I shall be going with "Plan B": JB Plastic Weld. A lot less money and a lot less work to achieve equal or better results.

What you do is go to the dealer with your VIN and have him/her/it/mutant give you the correct FORD P&A Service PN. If obsolete (and cannot find on parts locator system), you GOOGLE the PN to hopefully find NOS (evilBay) or cross the PN over to GATES or DORMAN.
Remember, most of the photos on these jobber parts sites are generic and may not appear as the correct part. You need to actually see the replacement part.

The FORD ONLINE CDP shows 6853 as the Basic Service PN.
Did the counter person show you (and printed off a copy) of the Parts Ill?
