I looked for 30 minutes and found a ton of threads about antennas but couldn't find how this one comes off. Does it screw off? I hope it does because it is impossible to get to the underside of it.
You will probably find that when you unscrew that Phillips head screw, that the mast will come off, but the base and part of the ball stays put. Then the base will most likely need to be unscrewed using some vice grips. If you can get that to unscrew (and be careful not to damage your surrounding paint while attempting this), the problem you will encounter is that the retainer and antenna cable will be on the underside inside the cowl. That area is really only accessible through the heater/AC fresh air duct on the RH kick panel, by reaching your arm up through the small opening. It seems impossible, but it's not, and is actually a good idea for cleaning out the leaves and debris that collects in the cowl.

I have traced the cable up behind the air duct and there is a hole in the cowl about 1 1/2" in diameter and the cable goes into that hole and attaches to the underside of the base. There is no way I can get my hand or fingers into that 1 1/2" hole but maybe I can fannaggle something

Unplug the cable from the radio, tie a piece of long heavy string to it, then tape the string in place to assure it won't come loose, then remove the antenna.
If you are planning to replace the "gypo" auto parts store antenna with a genuine Ford antenna, without the string, feeding an antenna cable thru that hole is a real b!tch!
The string tip is a great idea. Be aware that there will be a retainer for the base of this old antenna that will fall down into the side cowl, it probably won't rattle because of debris that collects there. This is the factory style antenna base:

Yes, the four screw holes need to be drilled on your cowl to use the factory style antenna. I'm guessing your truck was not originally ordered with a radio, or the holes have been filled when your truck was last repainted.
You have to un thread it, I am going to go take a pic of what it looks like under it.
You need to un thread the middle shaft, it is held on by 2 brackets. You can't really get in there to turn it, but you can get in there to hold it. Large flat tip screwdriver to hold the braclke, the un thread in the standard loosening direction.
