mine came already installed from the dealer
As far as the hitches that work for fifth wheel prep, I know of the B&W companion which I have and the Reese Elite.

If your dealer told you they don't come with the truck then he is either just a lazy ******* or a moron either way he shouldn't be allowed to be a dealer and not one that I would want to buy a truck from. But you can have your say you will be getting a survey shortly. Ford takes these surveys on dealers very seriously.

Curt and Demco also make hitches that work with the puck system.

In addition, Curt, Reese and Demco make adapters that allow you to use hitches that require bed mounted rails.

I went with Demco Autoslide as I have a SB and wanted some more cab clearance. After test hitching to camper I am glad I did, seems that center of Pin is further forward in my bed than in my 08' with a B&W companion hitch mounted in the flip over ball location. With the Demco though you have to be able to back up pretty much in line with the camper. Which I thought might be a issue but with tailgate down and camper hitched I have maybe 4" tailgate to 5th wheel clearance. So don't think hooking up at much of an angle would be very possible.
