When measuring from the centerline on the back of my cab to each frame rail I have 14 1/2" one way and 15 1/4" the other way. Is this normal? How do you align the box to the cab then, follow the frame or the cab?

No it is a left to right alignment that is off. Up and down I can deal with. I notice it when you look at how the box relates to the vertical lines in the back of the cab.
I believe the cab mounts are correct, did them twice, lol. Is there a difference between the left and right rear cab mounts?

I would check your cab mount bushings particularly in the rear. Then check your 4 front frame mounting holes for wallow (see pix).
These mounts will impact a lot of things including hood closing efforts and alignment.
BJ is right about the availability of the adjustable rear mounts but you should not need them unless your frame is warped. I would check everything else on the cab mountings first.
