Can you narrow it down to the area? If it sounds like its in the Trans area and by what you described and the engine has been out, I'd double check all the torque converter to flywheel bolts.
If the new driveshaft doesn't fix it, that's the next move.

Based on my cursory review of the thread, you sir, are the first to get the correct (probably) answer! I believe it was the carrier bearing. The new one piece drive shaft was installed today and all of the noise is gone as is the shudder at start off. I'm sure the pinion angle was off and whenever I let off the throttle, that angle changed and caused the noise. If the throttle was reapplied, it stopped. It all could possibly have been fixed cheaper, but that shaft had over 260k miles, had been through a major wreck (the donor truck was totaled, and had been messed with to try to get it balanced. It may be that it just would not ever balance no matter what you spent on it. Thanks for the help.
