I bought a diamond eye dual 4" stainless. Was delivered for under $500. The single and aluminized versions are cheaper. The one for the 94-97 powerstroke is the one i got, dont see any reason why it wont work with your body style, might have to add a mount or two. If you get the cat-back with off-road pipe, should tie right into your downpipe with an adapter and away you go.

3" straight pipe exit before the tire. 8' stick of pipe, mandel 90* bend, a few clamps and hangers... About $100 and a couple hours to do it. Local shops wanted $500-$700 for the same quick simple exhaust.
Over the axle will be a bit more work and may be worth an off the shelf kit. I did not go that route as I have a 38 gal rear tank, which was the reason I cut my old exhaust short of the axle. A little tight back there but there's room to make it fit if you wanted, just wouldn't necessarily be an off the shelf prebent part.
