The air pump on my 81 F250 is making noise. The engine is a 400ci, Where I live the truck is smog exempt. Would it be better to replace it or just remove it. If I sould remove it what is entailed. What will I need to do to make the truck run without it?

The smog pump injects fresh air into the exhaust gas, downstream from the valve in the cylinder head exhaust port. It has no effect on how the engine runs, so you don't need to change any tuning if you remove it.
If your truck's GVW is over 8500 lbs, it does not have an exhaust gas catalytic converter, and the only thing the smog pump does is produce a slight reduction in HC emissions (and aggravate closed-throttle deceleration backfiring).
If your truck's GVW is under 8500 lbs, it does have a catalytic converter, and it needs the smog pump to make the cat work.
Running a cat without the smog pump will prevent the cat from catalyzing CO and HCs (increasing emissions of those). It will also keep the cat from getting hot enough, and eventually lead to it's clogging and failure.
