2003 F350 6.0 with 290k+
It has a skip in the engine at idle and running. At times when running down the road skip seems to get worse.
Changed both fuel filters, didn't help.
truck start up is normal, no leaks, and flashing no codes.
Any suggestions?

What are you reading codes with? You can have pending codes without a check engine light.
Thanks Mustang 309 for the tip. You were right, there was a problem giving no code: #3 injector.
Will have the all 4 on right side replaced.
Also have the check the dynamic fuel pressure ( fuel pressure at wide open throttle under load condition ) it needs to be greater than 45 PSI or injector damage can occur, the further below 45 PSI the greater the chance of having injector damage. Low fuel pressure is the leading cause of injector damage. It would also be a good time to do the fuel pressure regulator blue spring upgrade.
