Well, after sitting for several hours you should hear the fan moving plenty of air at startup til you get the RPM's off idle for a few moments. This is because the clutch fluid has settled out of place and is partially locking the fan up.
Then you'll hear it less and less as it starts to freewheel while you drive and the fluid settles into place. Once the engine is up to temperature and the 'stat opens you should be able to let the truck idle with the a/c on without overheating. When you rev it up again you should hear the fan pulling air again, if it's working right.
