ICP sensor problem maybe?
could a bad icp sensor cause blue smoke and rough running on a cold start? it will seem to idle ok but if i give it some throttle it will be very rough. also there is a strange sound that comes from the exhuast that you can hear if you stand behind the truck right after starting it that will get slightly louder or seem like its getting worse until it stops completely and runs just fine. only take a matter of seconds to a minute or two before it starts running perfectly fine

Sounds like bad injectors. Yiu can check this with a feeler gauge.
Pull the valve covers
Pull the solenoid of the injectors
You'll see a square plate.
See what he bigges feeler gauge will fit under it
If .002 or less then you need injectors
