Printed on the label on the module which is *usually* up under the dash above the steering column.
Get a flashlight and look up there, also up behind the glovebox if you don't find it on the driver's side.

Its on A card in the owners manual, they come standard unless someone changed it 11331. If it's not there and that dont work you need to find the VATS module and it will be on the sticker, drivers side on top of the steering column toward the heater box.
