Transmission problem

My 92 3.0L with a 5sp manual trans has been having trans problems. It's most notable when i am moving from a stop i let the clutch out and push the gas and the engine speeds up but the clutch seems to be slipping it delays then it'll slowly start to roll and it's really bad on a hill, i'll floor it and it really struggles to get up the hill. I'm thinking the clutch is probably worn out but i was wanting to see if theres any thing else i may have overlooked.

Check the position of the clutch pedal at the point that it starts to engage and pull the truck. It should start to engage closer to the floor, rather than at the top of its travel.
If you can release the clutch pedal almost all the way before it starts to engage, it is likely the friction material is worn out.
You should be able to stall the engine with the parking brake set, and letting out the clutch. If it continues to slip, it's a goner... Avoid hilly country until you can get it replaced. I think it is easier to remove the shift lever & linkage under the truck than it is to remove the seats & carpet to allow the lever to come out with the transmission.

The clutch pedal does move almost all the way out before it starts to grab, so im gonna replace the clutch. When i do should i just replace the pressure plate too, or just the clutch. And do you guys think a clutch from autozone is good or should i get a better brand. Thanks for the help.

The surface of the flywheel and the pressure plate are both likely glazed and heat-checked. The flywheel can be re-surfaced by a good machine shop, but the pressure plate would be more readily replaced by a new unit.
I cannot recommend any particular brand personally. I'd check to see what NAPA sells, as they are/were a source used by independent mechanics in the past. They don't like to do their work twice, the second time at zero pay when parts fail prematurely or don't work at all.
OTOH, you can check out the listings at the various places. You can even find these parts on amazon, and ebay. If you plan on keeping it forever, relatively speaking, get a brand name you recognize, such as MOOG {and I don't even know if MOOG sells clutch parts}, maybe Raybestos? I just can't say. I have heard of LUK previously...
