It isn't the rim size as much as it is the tire diameter. Just because someone has 18 inch rims does not always mean that they have larger diameter tires.
I guess I asked the question wrong. I understand the tire size is not dependent on the rim size. I meant to ask:
1- does putting on a spare with a smaller diameter than the four wheels cause damage?
2- what do people do when they want to carry a larger overall tire diameter to go in the space for the spare.
I could measure the space but there is a exhaust pipe there so I just thought I'd ask those with more experience for ideas.

1. It can damage the clutches in the rear LSD. I personally would swap the spare to the front end if I had a flat on the rear.
2. I was able to fit a 315/75/16 on a factory spare rim under the truck with stock exhaust. It was tight! With 4" exhaust it no longer fit on my truck.

I wouldn't worry about any wear on the LSD clutches provided you are not driving it on a long trip with a different sized tire on the rear.
My spare tire is a 31.8" tire. The tires on my truck are a 34.1" tire. My solution to the spare tire size was to do nothing because it is a waste of money to put a bigger spare tire under the truck.
You will be fine with a smaller diameter spare for emergency use. (Think small donut spare tire on a car). I would not drive far with it though, just so you can get somewhere to get the truck fixed.
