Need tranny help. Weird/rough shifting

I have a 96 F-250 460 auto. When I shift from park to drive the truck will lurch forward. As it shifts from 1st to 2nd it shifts really rough, 2 to 3 rough but not as rough and 2 to 4 almost normal. From there it will not shift up anymore including overdrive. Anytime I try to drive over 35 mph the engine starts revving so high I am scared to take it any higher.

The two problems I am looking to solve are:
1. Is there a way to fix the rough shifting/lurchwhen putting into or getting out of park?
2. Why will the truck not go into any higher gears and overdrive?

Does anyone have any ideas what I should do to deal with this or where I should start? thanks so much!

That transmission is completely computer controlled so you need to pull the codes.

Sorry I'm not much of a mechanic. How would I go about pulling the codes? Are you talking about codes a parts store could pull for me or would I need another type of scanner?
