rough running 6.9

rough running 6.9
Hey all, a buddy of mine has a 86 f250 with the 6.9, and it started running really rough the other day. The truck will hardly go 40 mph. we changed the fuel filter and the typical leaky return lines and injector o-rings. At start up it shakes really bad and pukes out tons of whiteish blueish smoke. We are hoping its not the i.p. any ideas?
