My buddy has a 04 250 with nothing done to it. A while back he got a gradual nasty squeal in relation to turbo spool, pulled it off when it got real bad, and the shaft had some crazy play in it (im a gasser guy, I assume that's terrible). He swaped it out for a used turbo and now it sounds like its always trying to spool (no matter the engine speed or load) but its loud and nasty (sounds like a constant blow off valve, and the truck has almost no power. Took me 30 seconds to get it up to 45mph, it shifts hard as hell, and can't get it above that. I've been looking around and most places seem to say something along the lines of incorrectly reinstalled, hes going with a bad turbo. Any ideas here?
Sound like te replacement turbo is stuck. It's a variable vane design and they corrode from the heat and exhaust gasses. Not a big deal most of the time. Shifting issues should straighten out when you get the turbo working right. Here's more info.

Let it cool off and then crank it and feel around the large pipe on the back passenger side (quickly before it warms up ) you should feel NO air back there. Or on the smaller one on the driver side either.
