The truck has new rad, waterpump, t-stat, hoses, rad cap! The truck will sit and idle for hours no prob , the second you go for a drive she pukes all over herself !! I've went over everything twice . Tonight I discovered the vacuum advance is bad by disconnecting the end that connects to the carb and literally sucking on it nothing . It's like sucking air through a straw . Can this cause my overheating while driving ? Hopefully you guys post saying hell yes . I'm at the end of my rope with this thing .
As I was reading your post, I was thinking your timing may be off. It can have a significant effect, retarded timing especially. Fix that and check you temp sensor in cups of water with varied temps to make sure the indications aren't zero-way too hot without good transition.
When you say puking, how much? These trucks need a good 2in down in the neck for expansion, if you fill it back tight it will blow it. Is this what you are talking about?
Most times what you describe is reversed. These old trucks have enough open frontal area that keeping them moving forward 20 mph you can ditch the fan, but they won't sit and idle very long.

In jpbecker camp. As a reminder, the distributer vac advance line should be plugged while adjusting the timing. I could go into secondary vac. but would not matter. In addition to jpbecker suggestions; if problem persist, suggest place thermostat in pan of cold water and place over heat and monitor the thermostat opening, there's usually a noticeable pop when fully open. Anyhow, food for thought??
Yes, the vacuum advance just sucking air will lean out your a/f mixture and give you a hot/lean condition, also your timing is not advancing and adding to it. Replace vac, advance, set your timing, check overall advance and them go for a drive. If it still hot, as stated, check your thermostat, I boil water in the m/wave add a thermometer and drop in t-stat BEFORE installing in truck, I got 2 out of box failures before from kragen, now I check them first.

As the guys said;;;;; plus..... a plugged air filter can cause over heating due to a rich mixture, it is the same a leaving a choke on. Any rich mixture can cause it. If the secondary jets are worn out, as you come off idle, they will allow a rich mixture.
I keep my old trucks 2 1/2 inches from the top of the radiator, if I can touch water with my index finger, it is too full.
On the very simple side..... is the belt slipping and not driving the water pump/fan at the higher rpm?
