After installing Banks headers and full exhaust, I took my truck for another test drive this morning. SES light came on about 15 mins into drive and is telling me P0174 - Bank 2 Lean. SIGH!!!
So that's the right/drivers side - my list of things to go check

No wonder it was so hard to start last night and why I threw that code this morning - it does help to reconnect both rear hoses to the air intake. Code cleared and test driven again - didn't come back.

Any leaks after the MAF ?
Check the easy stuff first. This was why it was hesitating I think.
How old are the O2 sensors ? Try swapping them first if you go that way.
EDIT: I see you got it resolved. Good.
What do you mean BOTH rear hoses ? There is only 1, right ?
I believe she is talking about the two 5/8th maybe 3/4" hoses that go into the back of the intake
Yep the two at the back, larger one was connected lower smaller one was just laying on top the engine. Head smacking moment on why she didn't wanna run at start up yesterday. Surprised I even got her going, but she's all good now.

Amazing. Keep in mind I had 5 broken studs before I even started, and a flange that was hanging on by one last hunk of rust, all of that was pouring exhaust, was running rich and was a complete dog. Today after I got the intake connected correctly I went to pull out off a side road onto the main road - paved but was a pile of loose stuff and I absolutely peeled my rear tires. Never done that before and I wasn't even trying, just used to having to stomp it to get her to go, I giggled for a good mile or two. Looking forward to installing my tunes later this week.
