If the motor revs up but the truck doesn't accerate that's a slipping clutch or transmission.
Cwelchy to me your problem sounds different, in your case the truck builds speed no problem but then bogs down again. That could be a case of too big a tire and not enough gear, or an internal trans problem affecting 4th gear only.
Mine does this with every gear. OP stated that his will rev to ~4500 and then not accelerate. I am assuming that he is estimating since auto trans vehicles typically do not have tachs. Maybe OP can eloborate.
Maybe put a fuel presure gauge on it and take it for a drive, if pressure drops as you're accelerating the pump is weak or the filter is clogged.

Yeah doesn't matter what the rpm is really, a reving motor with no acceleration is a slipping clutch.. somewhere.
i have a 95 f150 5.0 4x4 4r70w trans. normal tires. It turns out the truck runs fine. I guess. I have a good friend of mine drive it around for a little bit to see what he thought. he said that it was driving the way it should. he is alot easier on the pedal then i am but said that it was gaining speed just fine. Shifting down when needed and shifting up when needed. To me it still lacks the power of what i remember but he says that its all in my head. Thats fine its cheaper to keep it in my head then have to tear one apart to fix something. One of these days when it warms up i will play with the engine more and see if i can find out where my lack of power is or just give it more to play with
