I have searched and cant find an answer to my particular problem. i have a 2007 f250 6.0 and it starts hard when cold and continues to run rough when idling until warm. i bought the truck with 107000 miles back in the spring and changed the oil and fuel filters at 112000. after the oil change it immediately began to run rough when started cold. throughout the summer months it ran rough when first cranked but as it has gotten colder the rough idling and hard cold starts have got much worse. i am within days of trading the truck if i cant find an answer to the problem. i dont think it would be an FICM issue due to the fact that the rough cold starts and idles started immediately following the oil/fuel filter change. I have also noticed recently that when cold the glow plugs dont make their usual sound.
Some of the common cold start issues on the 6.0's are due to injector o-rings cracking and leaking. Most commonly o-rings on the plugs on the end of high pressure oil rails leak and cause hard starts because of the lack of pressure and when the motor warms up they seal and then it runs fine. The easy way to know if your losing pressure is to use a scantool that shows the IPR % and the ICP pressures. You should have while cranking atleast 500psi but if everything is sealed properly it will be much more that that. You should also see 84% open on the IPR. also while cranking you should see the FICM reading 48V or very close to it.

When you did the filters what oil filter did you use?
The two things t hat happen is the wrong filter is used of an
aftermarket cap with the right filter = problems.
What oil was used? And to say it's not the FICM you need to test it
and that can be done with no more than a volt meter and a helper.

With the 6.0L it is best to use genuine Ford filters. Aftermarket filter manufactures claim to produce their filters to OE specs but in some cases it is not so. Also you should use Synthetic 5W40 oil to keep the motor cleaner and reduce stiction. If you took you truck to a dealer it would be between $1,500 and $4,000 depending on what they wanted to replace.
Fram has changed the design of there filter and is not a good option
any more. And I don't know if the Napa has the right water cover of the 1st stage.
I have found you can get a set of filters at you local Navistar dealer for a lot less
than from Ford. With the problems starting right after the change I would go back
to OEM approved filters and get someone to scan the codes for you. You need a
starting point and not just tossing parts at it.

If you want to keep the cost down on the filters you can get then from
a Navistar dealer aka International. How ever you do it OEM spec is the
only way to go. Fram is no longer a good choice do to the fact that that
have changed the filter that they have. Rotella T6 is a good oil to use and
yes that is what I use. I get my filters from Ford or Navistar. The problem with
the non-OEM filters is you can get unfiltered oil into the system and much up
the injectors and then you have a very unhappy engine.
