Im not sure what you mean by the right breather looks added.
The valve covers look like a matched set, both have the same patina and both have the same tabs for the plug wire holder.
The breather on the cover with the pcv is a push in and the other cover has the tabs where you push in and twist.
There is nothing special about this truck, its just one of many.
The breather on right valve cover that you say is a push in. Does look like its been there for a long time but not stock on a 65-66 F100-250. Have not looked it up but possible on a larger truck. In pic it pretty much look like it belongs with the patina. All I am saying is not stock factory on that truck.
Same with my '65 There are a few things original owner added that are not correct or right but I left just because
Nothing stirring the pot there . . . PCV on the 352 in 1966 but no breather to air cleaner hose unless it was california. 1967 totally different animal.
I think my 65 service manual has a pictorial of the 352 engine with the same setup as the 67 of Mr Hellfires. Ill have to get it out and take a look if it does ill try and scan and post. Ha Ha Randy you shot me down thought I had something there.

your spoon must have broke, the stir did not go well!
The '66 Shop manual shows both system's. California and the system of all others. Just look at about any 352 air cleaner and it does not have the fitting~nipple to go to breather. Good drawing in the manual.
Two systems : Open and Closed Just like this case: Open & CLOSED! LOFLOL

Lets see that breather tube pic out of '65 shop manual! Ok, that was a attempted STIR !
hows the rain up there? I mean the sun, do you ever get to see it? About the only person that lives in spokane that gets to see the sun is a airline pilot and crew!
Still 70's and sunny every day here. Low 80's last week. Still have not had to turn the heat on in house. . . Actually open the doors for a bit in evening to cool the place off.

Take a look at the '66 shop manual . . . Shows pic of engine with a
R O A D . . . D R A F T . . . T U B E ! Haha where is ND when ya' need him.
Im still trying to figure out where my covers belong.
Additional information:
Boss my covers are just like yours except the breather on the cover with the pcv has a tube about 2in high that the breather pushs on. All three holes look factory orginial, I do not believe that any of them were drilled by a PO.
On the lower part of the block there is a tag rivited to the block that has the Ford logo and it says "remanufactured engine". There are numbers but I cant get close enough to read them.
It looks like the engine and valve covers were black, there is no evidence of blue paint anywhere.
I have surfed the web for hours looking for FE valve covers with 3 holes, I cant find any at all.

Can you post a pic with breather off showing the tube? My guess is the valve covers are from a 361 or 391 Large truck, possible someone added the 3rd breather. WIth a pic could probably tell better. The way Ford intended the system to work it makes no sense having the 3rd breather. Looks neat and like it belongs but do not think its stock ford for a 65-66 F100-250
Maybe the reman engine is out of a big truck and those valve covers came on it? Check the stroke to see what it is.
My guess is prior to 1966 no matter what model its from.

Looks like an original set of '65 valve covers vs your '66 truck. Gold is the giveaway. Intake is blue valve covers are gold and look like the original color~ford and not redone. I still say 1965 Big Truck and for sure not original to your truck.
What is date code on intake manifold, heads and block? Casting numbers?
Block date code will be where oil filter adaptor bolts up
