Then truck presently has a clunk in the drivetrain. It's noticeable when toggling between drive and reverse. There's no excessive movement of the engine, which indicates the motor mounts are fine, and he says the u-joints are fine. I guess that leaves the transmission or rear axle.
Crawl under, or alongside, and watch the show as the selector is moved from gear to gear. You will likely see the driveshaft move a bit in either direction as all slack in the drivetrain is taken up. Unless it is severe, it is not a major source of worry. It would be a good idea to check the differential for proper lube & level. FWIW, I have an old one that had never needed transmission lube added every time I checked. Took it on a trip, and it, or something, started to smell hot as I neared home. The transmission was down, and I assume that it was a very slow drip from the speedo cable adapter, which has leaked since it was new.
