OK, I have been gone for a while -- just running the truck. I have driven the truck several hundred miles since clearing the codes. Since then I have not experienced this stumbling upon acceleration, and I re-checked the codes today and there were no error codes, just the pass codes. It's a little frustrating nothing has resurfaced, because I didn't really "fix" anything other than replacing the fuel filter.
If something comes up I'll post it here. It would be nice to have a clear resolution, but I suppose I should be happy I'm not having any problems.
For the sake of completeness I wanted to finish this thread. I replaced the fuel filter (as described) and didn't encounter any more truck dying issues; I suspect the fuel filter was simply clogged with garbage pulled out of the broken (and rotten) rear pump. Fine.
Vacuum was not as was to be expected and I heard a sucking sound when pressing on the brakes. Replacing the brake booster fixed the vacuum problem and improved performance a bit.
Lastly, the truck would always start strong when cold, but if shut of and restarted when warm it would not always run as great; it would run rich and crappy. Also, I was getting a computer "Code 18 is telling you the computer isn't getting the correct signal from the distributor pickup (aka stator, a hall-effect device)". I replaced the distributor (module) and this resolved the final issue.

Glad you got it running good. Ive had mine running good for a year or so now and it is very reliable, starts well in the cold or hot.
