A tuner by itself just uploads new settings (known as a "tune") to the computer that controls the engine and the transmission.
A "tune" that the tuner uploads can change many things. Spark advance, amount of fuel at wide-open-throttle (WOT), shift points, and shift harshness.
In my case, on a 2001 2-valve V10 w/automatic, he shaved a full second off my 0-60 time, on 87 octane gas. It was a combination of engine tuning and transmission shifts.
The tune makes a big difference in terms of throttle response, overall power, and shifting, and Mike can adjust his tunes to whatever you think you want. Don't like it the way it is? Explain it to Mike and he'll fine-tune your tune (pun intended).

If I'm not mistaken one of Mike's tunes does not affect the existing factory warranty. Naturally this is something you'd want to check but Mike wouldn't steer you wrong about it.
The flash device swaps out your stock tune for a 5 Star tune then stores your stock tune on the device. The device is then locked to your vehicle until you install the stock tune again. Hands down the best mod (plug and play) that is out there. It changed the whole feel of my truck. I dont tow a heavy trailer maybe 7-8 k but it made a huge difference with that. Stock, the truck seemed to shift and search for gears on the highway and up short hills. Not to mention the extar power. The tunes add tons of power. It is truly amazing that there could be that much power for Ford to leave out of it with a simple tune.
Now 5 Star, is the best to deal with period. They will answer any questions promptly as said previously. A completely custom mod with great service and now it's only$389? I wish I had another truck........haha
