I'm assuming that you need the flange that connects to the driveshaft with the U-joint and then bolts to the flange in the picture.
According to the Ford parts catalog there are two possibilities. One for the 1270/1310 U-joint, the other for the 1350 U-joint
1270/1310 U-joint
B9T-4866-A
Aitkin Motor Co., Aitkin MN has 1 (218) 927-2169
Griesel Motor Co., Okarche, OK has 1 (405) 263-7242
1350 U-joint
D3TZ-4866-A
Green Sales, Cincinnati, OH has 15 (800) 543-4959
Van Cleve Ford, Morton, WA has 2 (360) 456-5999
If you want to double check and make sure it's the right flange here is the page from the Ford parts catalog giving the flange dimensions.
