What is hard idle? Discribe it. Cold starting, hot idle, throttle response etc ., what is the rpm and how stable is it?
What do you have?
Have you checked for any codes set with a scanner?
Until you get some indication, it's just guessing.

Ok, you got fuel pressure. So how do the plugs look? All of them. Are they wet? What is the gap on each of them. If you come back quick I'll know you're blowing smoke up may a$$ because the rear ones are major PIAs until you get the hang of it.
If you can, how about doing a compression check on all of the cylinders while you have each plug out (this will tell us if someone futsed up the timing and bent a valve or something).
Interesting that you have a fuel pressure tester. Your tool set must be pretty good since most people wouldn't have one.

I'd ask Bluegrass if crank position sensors are an issue with these. I've never had one go bad but I only have one 200K mile 5.4 to judge by with these trucks. I generally have my hands in Jags and they have an issue with them.
