Is a 6.0 with 275k miles and no known head gasket/ oil cooler/ egr cooler issues, ever, a good thing or an accident waiting to hapen? I have looked at at truck with stated 275k miles and owner claims to have never had any probs with the known 6.0 issues.
I'm at 158K on my 2003 6.0...one of the real early 6.0s and I've not had a bit of trouble out of her. I pat her dashboard weekly and tell her what a grand ole girl she is....and she just keeps purring along!
At 275k, ANY vehicle is going to need stuff. That doesn't mean the "known issues" are going to rise up, my truck has 210k and never an issue. However things like ball joints, cv boots, axles, brakes, rust underneath, would be something to pay attention to. Two ball joints will certainly set you back a grand. Even the steering rack could need service. What if he never changed the trans fluid? Or the rear diff? If everything was properly maintained, and hte price is reasonable, I'd say go for it.
More miles just means that there's an increased chance of someone not doing proper maintenance at one of the schedule intervals, simply because more intervals have passed.

Our trucks DO NOT have CV boots nor a steering rack! Man you gotta be sore! You pay $850 in labor for 2 ball joints you gotta be kidding.
The axle, it doesn't have a boot over the joint?, and the "thing" that steers that gets loose and periodically needs adjusting by a bolt. My terminology isn't great sometimes.

If your periodically adjusting your steering box, it needs replacing asap unless your life isn't worth the money.
To be fair, there ARE likely LOTS of 6.0s out there that still have the original EGR and oil coolers as well as never having the heads lift. It's just that we don't hear of, or read about the non-problematic ones because the owners are obviously not going to say anything or complain.
Okay, so we know this is a 6 speed manual truck. What is the year? Is it the original owner? What is the asking price? What is the maintenance and repair history of this truck? What's the overall condition of the body, interior and mechanical components? I'm assuming the point of this thread, is that you are intending on purchasing it so you seek advice, am I correct?

Searching just basic options with single rear wheel that's actually a very fair price.
