I have a 2001 Ford Escape v6 and it is reading misfire on #4 and #6. It only happens intermittently. It already has the cats cut out and it has a brand new EGR so they aren't clogged. I have seen where it might be the PCM, but it had the recall work done on it many years ago. How do I prevent this from occurring again?
Pull the COP's and check for oil in the plug area. Oil is a dielectric, but with contaminants, it becomes a partial conductor. The plugs can fire to the head rather than inside the combustion chamber if there is oil there.
The COP units can also fail. The plugs need to be replaced or re-gapped over time, have you done that? If the gap is too large, they misfire, and can take out a COP {$$$$} or a computer {$$$$$$$$$$$}. Plugs are relatively cheap, around $2.50 each.
If there is oil, you can replace the O-rings that seal the plug tube, and may solve the problem.

Several other things cause misfire. Injectors clogged or leaky, fuel pressure too low or too high, leaky valves or rings, and the ignition system including the COP and the boot on the COP. The wires to the ECM could also be intermittent if they have been flexed too many times or are corroded, or even the ECM could be failing.
You can replace or rotate the COP and see if the misfire follows, check the resistance on the wires to the ECM, inspect the boots for indication of spark eating a hole in the material, check the fuel pressure {pump and/or regulator} and delivery volume, though it would cause misfire in the 'weakest' cylinder, and maybe all. And check the compression.

I had the same problem, the spark plugs weren't getting the high voltage, replaced the faulty ignition coil and boot on that particular cylinder. As cars get older the high voltage insulator break down.
