Great list and my first post.
I have a 93 E250 van with quigley 4x4 conversion
The 4x4 allows ABS on the rear only. Otherwise I believe the brakes are standard Ford truck.
My brakes work fine and they have been receintly flushed and bled.
The problem is how fast the brakes react. If I stop normally - no problem. If I apply the brakes suddenly the pedal will "go hard" and the brakes do not apply immediately. There is a slight lag. The lag is such that emergency stops are not as fast as they could or should be. The pedal initally goes hard and then sinks into braking mode.
Any ideas?

I would check the power brake booster first, it sounds like it's a good place to start. Maybe a plugged vacuum line (or hydraulic line if it's hydraulic assist like some GM's). Maybe this'll get you started in the right direction.
