Any reason to use a relay or anything ? I have a 20a red lighted toggle switch laying in my tool box that I can use....
Also (not an electrician) the switch has 3 prongs , common ground (any ground bolt I'm assuming) and "on acc" and "supply volts" Am I correct that the white/blue goes to "supply volts" and Light blue/yellow goes to "on Acc" ?

You don't need a relay, your just sending 12V as a "signal" to the PCM to idle the engine up. So you don't need a relay. Sounds like you've got a '03/'04 truck (the wire colors are slightly different in later years). With that said...
Ground terminal...there are two good ground screws under the steering wheel; behind the removable panel that you've got to pull to get at the fuse box.
Supply 12V (only hot when the IGN is on)...you want to use white w/light blue stripe wire hooked to the "supply voltage" terminal of your switch.
Accessory (to activate high idle)...the light blue wire w/yellow stripe.
