Broken piston skirts

After having a broken piston skirt on #5. I have been trying to find the cause of this. I have found others that have had this happen to them. But haven't found a definate answer to what it caused it. Have heard, to much boost, piston cooling jet plugged, lot of hard high RPM running, running it hard with cold engine. The list goes on and on. Obviously these could be causes. But is there one correct one?

Broken skirt is usually caused by too much piston-to-wall clearance or defective part.

I have only found them on heavily worn pistons cooling jet problems usually show up as burnt up piston tops at least that's been my experience!
