Can't speak for the 2004 models, but my 99 did almost the same thing and it turned out to be a broken shaft on the blend door in the under dash assembly. Took me 2 days of lying upside down to fix it.
Hopefully yours will be something simple like an actuator motor assembly or vacuum leak.
