Hard starting 5.4L?

My 98' F150 5.4L 4X4 with E4OD has been working great considering the 186k plus miles on it. Yesterday while driving around all warmed up. I came out of a store we went into for a couple minutes and the engine turned over and didn't catch / start right away. The second time it eventually started and seemed to run fine. We stopped for another short errand and this time it would not start until the second or third try and then at the end of turning over a few revs. Latter on I tried it again and it started right up. Today once warm, it started every time right away. What do you guys think? Is the fuel pump going? I had the fuel filter replaced about two years ago. Please elaborate on all the possible causes you think may be at fault here.

concidering that it is Hit/miss...start by checking vacuum lines for leaks. & cleaning the MAF

I clean the MAF about a year ago and the intake emission components about 6 years ago: IAC, EGR, Throttle body, etc. Time for another cleaning? I have replaced a few of the vac hoses, but will check the rest for degradation as you advised. I have never replaced the OEM DPFE?

There is also a small rust/ pin hole in the top of the gas tank that throws an occasional code; .04 in Vac leak every few months and there is no more Vac release sound when the gas cap is removed like there had always been in the past? Ignition COP's and plugs are all new Motorcraft installed about two years ago. What are the most common problems for this hesitation in starting

Check the cable going to the starter. Mine has corroded and broke twice on me now.
