Starter Replacement
I was told I would need a special wrench to get to the top bolt. Is this true?
2003 E-350 7.3 diesel original starter with 657821 miles. Engages then disengages and spins. Takes several tries to finally start. Before I buy the new starter, want to make sure I have the correct tools.

Nope, you can use a standard wrench but you have to keep flipping it, takes forever. I've had good luck with a swivel head ratchet, the craftsman is the lowest profile, that and a short 6 point socket, angle the handle back and you'll have 2-3 clicks worth of swing.

You might want to find a good rebuild shop to have your starter rebuilt instead of getting a poor quality replacement. There seems to be lot less than junk out there being sold from large suppliers that wont last as long as your original. Do some searches on this forum and read up.
