jokerforever: I ordered the camperpackage on my 2011 F-350 cc and I didn't get a certificate with mine and I am not sure that a lot of other owners didn't get one either.
The "Slide-in camper certificate" is listed as part of the camper package (option #471). There are a few items which are often carried separately from the vehicle while it is transit to the dealership, and they can be misplaced because of this. If you do have the camper package, and you need/want the camper certificate, I would suggest revisiting your dealership to have them make it right.
I had something similar happen with some of the add-ons that came with my Harley-Davidson Super Duty. In the end, the sales manager ended up having those things ordered in order to provide them. (Personally, I think someone at the dealership snagged them for some supplemental eBay income.)

Yeah, it may have been misplaced. Or more than likely its folded up in the back of your owners manual. Take a close look, if you don't have it then you really should talk to your dealer. It's part of the package.
