My antifreeze is sludge!

My antifreeze is sludge!
One of our fleet vehicles here at work came in for service today. Our mechanic was changing one of the plugs on the end of the head (leaky o-ring) and noticed the antifreeze jug was a little dirty. When he drained it, there was about 3" of sludge in the bottom of the fill bottle!
Anyway, he is flushing the system and replacing the hoses now. I was just curious if this is caused when you mix two different kinds/colors of antifreeze?
We have a lot of CAT equipment around here, and I am wondering if one of the service guys accidentally put the CAT red antifreeze back in the 7.3 when it was flushed last. I think these trucks came with green. Atleast that is what I recall on my personal truck. Am I correct on any of this?
Please enlighten me.... Thanks in advance.
