Couldn't the groove be caused by rocks or dirt getting between the pad and the rotor? I guess it doesn't even need to be a rock just plain dirt would do it. I know on our chevy work trucks everytime we take them out on gravel roads they end up squeeling do to the dirt. I wouldn't think the dealer would warrenty that.

In my case, the rotors got grooves on both sides, and warped just a little as well. The dealer asked no questions and handled the issue better than I might have expected.
Only problem is I now have front rotors that have been turned once, rather than new rotors. But, I did get new pads and calipers.
As a side note, the brakes prior to the work just never seemed to feel 100% "firm". Sometimes felt like there might have been a little air in the lines.
After the work was done, that problem is gone.

And I do think there is mroe info in the Brakes forum. You'd just have to search it and see.
I seem to remember the service manager mentioning that my rotors had a green dot on them indicating I had the new style rotors.
