Here you go, these guys make great products from what I hear. A buddy of mine bought their Cummins to ZF6 adapter plate so he could get rid of the awful G56 in his Dodge, and from what I saw, it was a well engineered piece.

Trick question, the price will depend on the year of the truck and tranny type (auto's cost more), and depending on what Cummins you want to use.
A 12v Cummins conversion is much cheaper and simpler than a 24v.
I've seen turnkey 12v kits from 1-2k, plus the price of the motor (4-8K).

I have heard the swaps can range from 6k to 20k depending on engine transmission...etc etc. A manual trans will help make it a little cheaper but still a lot of money. A 12V (ve or p-pump) will be your cheapest options and a 6.7 would be the most expensive.
I would LOVE to drop the 04.5 out of my old dodge in my truck but that ain't happening.
