Welcome to FTE, The hard starting is either your GPR or GP them selves. You can try jumping the 2 large terminals on the GPR for 30 seconds and see how it starts thenm

The white smoke and loss of power that you are describing leads me to be a turbo issue, it could be as simple as O rings or as "bad" as a blown turbo. Have you checked the wheel shaft for play?

Before you go as far as pulling the injectors, (Although I think the guys are right since you didn't mention a check engine light coming on), check your wiring. There are some known issues with the wiring for the injectors.
Since you will need to pull the valve covers anyway to get that far, check to make sure that all of the injector wires are plugged in solid. The wire catch should be over the top of the injector. They plug in from the bottom up then the wire swings over the top to "lock".
Check that none of the wires under the valve cover are rubbing against anything that moves causing a short. They do have a little room to flex but you can move them clear of any moving parts.
Check the plug that goes through the valve cover gasket to make sure it is seated 100%. This is the harness for both the injectors and glow plugs. If the aren't the "new" harnesses, then put the little wedge pieces from Ford to make sure they don't come unplugged. Or you can do the 50 cent mod. (Search for the 50 cent mod on this forum for details). I just prefer the little plastic wedges or replacing the harnesses.
Check the wire harness that goes over the drivers side valve cover. known to rub the valve cover and wear through the wire insulation causing a short.

If the above doesn't take care of the problem then I would start with the earlier posts on the injector O-rings, then maybe injectors.

For the cold starts, if the wiring to the glow plugs check out, first try replacing your glow plug relay. Alot of us here switched to the big Stancor relay. About $30 and easy to replace. Will last alot longer too.
If that doesn't help then you will be looking at replacing the glow plugs. Although, since you are in the valve covers already checking the other stuff out... wouldn't be a bad idea to replace them since you are in there.

With that many miles, depeding on how it was maintained, the turbo could still be good. I have 250K on mine and it still looks like new and no play in the shaft. But I have replaced the glow plugs, the relay, injector harnesses and valve cover gaskets.
