I have blown off the turbo hose more than I care to say but never had this happen. Only thing I can think is it was too tight. Fortunately I carry spare turbo hoses and clamps.
That reminds me of the time my water pump pulley sheared off around the bolts.
I only ever torque the clamps to 96 lb-in but even that seems excessive. I had the Mishi pipes on my last 6.0, and their spec was 27 lb-in, probably to minimize deforming the thin wall aluminum pipes. Never had one of those blow off with stock boost, but I always clean all surfaces with paint thinner and denatured alcohol before reassembly.

I just bought an aftermarket 4 hose turbo hose set up as my stock original hot side slightly bent turbo hose had a rip in it. I put it in and took the truck, a 2003 f250 4x4, for a big boy ride and pop and no power. I am not familiar with the pop but popping the hood it was oblivious. I limped into a repair shop and the guy immediately said "you didn't use the Ford part!!". "The Ford hose has a groove to line up with the pipes tube". Another guy says "this happens all the time, you don't have tools?!?!"
Does these hoses pop off all the time?
The Ford dealer just told me these hoses are obsolete from Ford.
